one:
"Any endorsements?"

        my decent shows:
        endorsement ->1. żyro, indos 2. poparcie; aprobata 3. adnotacja na odwrocie
        (dokumentu itp.)

        Jeżeli to ma być adnotacja, to ja o takiej na odwrocie prawa jazdy nie
        słyszałem! Co by to miało być?

          endorsment - a note made at the back of a driving licence listing: date of the
          offence, when it becomes spent, poins incurred and fine paid. Non-endorsable
          offences are those that are not written to your driving licence (parking fines,
          for example).

      They are asking if you got fined for any driving offences; they (~UK) have
      different codes for different types of offences, you can look them up on:
      www.dvla.gov.uk
