Designation - What is it?

In an application form it is a word "Designation". What does it
mean? Is it title like Mr, Ms, or rather position at job like team
leader? Or something else... The word "Designation" appears after
blank place for referee's name, and it is also a blank place after
it, so I have to write there something.
Thank you.

Yes, you are right, 'designation' means name or title. So if you are
acting as a referee for someone then - under designation on the
application form - you would put your title for eg Miss Brown.

I am not acting as a referee. I am looking for a job and I have to
fill in application form. It is like:
        Name ________ Designation ________

        Is it right:
        Joanna Smith for name
        and
        Ms for designation

        What is more appropriate:
        Ms or Miss
        Ms or Mrs
I know exact, who is married who unmarried.

        Thank you

I see! In this case it would seem they want your job title:

Name: Joe Brown Designation: Marketing Manager
