I need big help

I worked in England and a come back to Poland. I need "Statement of Earnings",
but my language in wrote is not good. What I have wrote in mail message to my
firm?

Pracowałam w Anglii i teraz musze wysłać mailem prośbę o podsumowanie
zarobków, wiem, że to sie nazywa "Statement of Earnings"
ale pisownia mi kuleje. Pomocy.

I worked for (nazwa firmy) between (data rozpoczecia) and (data
zakonczenia pracy). I have now returned to Poland and need a
Statement of Earnings for tax purposes.

You might also be able to request a P60 from the IRS which would
have such information.

As Far as I am concerned P60 form is the only one official document
for tax purposes, not payslips or statement of earnings.
Unless in Poland accept other documents.
