I'm not sure if I understand...

Are you going to apply for a loan, or you want to design a form for such
application?

In the first case your bank should provide you with one.

In the second case just go to a search engine like yahoo.com and type magic
words: loan application

There will be plenty of hits... By the way: most banks have such forms on their
websites so just try to visit some...
