classroom language

I want to ask a teacher for help. Is it correct to start my question with:
"please madam"
or just "madam"?

      Hi Katia,
You can say 'Excuse me, Miss... but this is very formal and, if your
teacher doesn't mind, I think it's better to use her first name.

        Hi,
and thank you for so quick answer.
Well, in Polish schools the athmosphere is rather formal ;)
I'm curious, how English children call their teacher. Do they use her/his name?

In most English schools the teacher is normally called Mr...or
Mrs..or Ms.. or just sir..when I teach I always ask my students to
call me Jon as I prefer to be informal..:)

              It's a pleasure rather than a bother!
